چکیده

Chronic lymphocytic leukemia (CLL) is thought to be a malignancy of bone marrow-derived (B) lymphocytes, since the leukemic cells from a majority of patients bear easily detectable membrane-bound immunoglobulin (Ig) (1), possess a complement (C3) receptor (2), and do not bind sheep erythrocytes (3). Since in most studies (1, 3-5), but not all (6), the cells from an individual patient bear Ig restricted to a single light chain class, it is suspected that the CLL lymphocytes represent the progeny of a single clone of B cells. The present study provides support for the clonal proliferation of CLL cells by demonstrating in a single patient that the Ig on virtually all of the CLL cells possess the same idiotypic (variable region) determinants. The anti-idiotypic serum, prepared against a circulating paraprotein, reacted only with the paraprotein and the surface Ig on the CLL cells and not with a variety of other serum or cell-bound Igs.

The nature of the immunoglobulin G on the surface of B lymphocytes in chronic lymphocytic leukemia

The nature of the immunoglobulin (Ig) G found associated with the neoplastic B lymphocytes in chronic lymphocytic leukemia that also express Igm and IgD had been investigated by absorption studies using anti-idiotypic antibodies raised against cell surface IgM from five patients. In all five cases, although cellular IgM and IgD behaved as idiotypic, the IgG did not. Thus the IgG frequently foun...

Anti-idiotype sera raised against surface immunoglobulin of human neoplastic lymphocytes

The idiotypic determinants of surface immunoglobulins on B-cell lymphomas and lymphocytic leukemias represent tumor-specific antigens, individually unique for each tumor. As such they have both diagnostic and therapeutic potential, particularly for those neoplasms with no serum monoclonal immunoglobulin arising from synthesis of the protein for export. We describe the raising in animals of anti...

Extracellular idiotypic immunoglobulin arising from human leukemic B lymphocytes

The peripheral blood lymphocytes of nine out of nine patients with typical surface Ig-positive chronic lymphocytic leukemia but no paraprotein visible on serum electrophoresis have been shown by radioimmunoassay to export small amounts of pentameric IgM during culture (in the range of 2.4-7.2 ng/10(7) cells per h); three out of nine also exported monomeric IgD (0.7-1.4 ng/10(7) cells per h). Im...

In vitro idiotypic suppression of chronic lymphocytic leukemia lymphocytes secreting monoclonal immunoglobulin M anti-sheep erythrocyte antibody.

A patient with chronic lymphocytic leukemia was found to have B cells with surface immunoglobulin (Ig)M manifesting anti-sheep erythrocyte (SRBC) specificity together with a high titer serum monoclonal anti-SRBC IgM antibody. By immunizing a sheep with the monoclonal IgM antibody, followed by multiple absorptions against normal human IgM, an anti-idiotype (Id) antibody was obtained. The serum I...

Chronic lymphocytic leukemia, biology, new diagnosis and treatment: review article

Chronic lymphocytic leukemia (CLL) is a malignancy of B CD5+cells and is the most common type of leukemia in adults. The disease is more common in men over 50 years in western countries. CLL is associated with defective apoptosis in B cells. CLL was traditionally regarded as a disease that occurs before naïve B cells meet the antigen in the lymph nodes. Laboratory diagnosis requires white blood...
